Communication
As I make every effort to address your needs around healing, I hope that you as my patient will make every effort to follow through with our mutually agreed upon treatment plan. I ask that you inform me of all forms of therapy you are currently pursuing. You should feel comfortable letting me know if you desire another professional opinion, express your needs, and provide feedback so that I can better serve you and my other patients
Appointments
Your appointment time is reserved especially for you, and we do our very best to make sure that we focus the entire appointment on you and your needs. If you are running late, we hope you understand that we cannot extend your appointment time as that will take away time from the patient whose appointment is booked after yours. If you must cancel, please provide at least 24 hours notice, so that we may accommodate those who are waiting for an appointment.
Your First Visit
At your initial visit, please come prepared with the following:
- A completed Health Questionnaire (please click here to download and print)
- A completed New Patient Information form (please click here to download and print)
- Copies of relevant medical records/lab results. You may arrange to have these sent from other doctors in advance
- A complete list of all medications and vitamins/supplements you are taking – you may wish to bring in all the bottles
- A list of questions you wish to ask

- The insurance phone number for providers on the back of your card if you did not include it in the online scheduler already.
For those whom are self pay, the cost for the exam/ first one hour visit is $180. I accept amex, visa, m/c, cash, and checks.
Insurances accepted in network are: Aetna, Cigna, United, and Oxford.
